How to Change My Polling Place Address in Georgia

    • 1). Obtain a voter registration application, available online at the Georgia Secretary of State website or at your county board of registrars' office. County offices are listed on the Secretary of State website.

    • 2). Complete the form. Fill in your full legal name, physical address, mailing address and phone number. If you wish, you may include your gender and race. Fill in your full Georgia drivers license number or Georgia state issued identification number or the last 4 digits of your Social Security number. State whether you are willing to be a poll worker. Check the appropriate boxes indicating that you are over 18 years of age and are a U.S. citizen.

    • 3). Sign the form. If you are using the online form, you must print the form and physically sign it. Your signature indicates the following: you have read and are swearing an oath that you are a citizen of the United States; you are over 18; you live at the address provided; you are eligible to vote in Georgia; you have not been declared incompetent; and you are not serving a sentence for a felony involving moral turpitude.

    • 4). Mail or hand-deliver your completed application to the Georgia Secretary of State's office or to your local county board of registrars' office. Do not fax your application as it will be rejected.
