How to Report a Lost Immigration Card

    • 1). Report your lost green card to your local police station and file a police report. Doing this will prevent other people from misusing the card if they find it.

    • 2). Visit the official website of the U.S. Citizenship and Immigration Services, download and fill out the "Application to Replace Permanent Resident Card (I-90)" form.

    • 3). Sign the completed I-90 form, enclose a check or money order for $450 and mail it to: USCIS P.O. Box 21262 Phoenix, AZ 85036. If you want to mail it via express mail or private courier, mail it to: USCIS Attention: I-90 1820 E. Skyharbor, Circle S, Floor 1 Suite 100 Phoenix, AZ 85034.

    • 4). Wait for an appointment notice to have your fingerprints taken at your nearest USCIS center. You will receive the appointment letter about several weeks after the USCIS receives your I-90 form. After receiving the notice, proceed to the USCIS center and bring the appointment notice, alien registration number and your passport issued by your country.

    • 5). Call the USCIS National Customer Service Center (NCSC) at 1-800-375-5283 about one to two weeks after submitting your fingerprints to find out how long you will have to wait for your replacement green card. Have your alien registration number and passport number handy when calling.
