1Obtain Form 990 Schedule A from the IRS website or request a paper form from the IRS.

Choose the explanation on lines 1 through 11 that best describes why your organization should be considered a public charity. Use your exemption letter as a guide if you are not sure which reason to choose. If you believe you qualify as a public charity for more than one reason, choose the best explanation and mention the other reasons in Part IV of the form. Check the box next to the reason you choose.

Complete Part II of Schedule A if you checked the box on line 5, 7 or 8 of Part I. Report all public support in Section A and add total income in Section B. If your organization has been tax-exempt for five years or less, check the box next to line 13. Do not fill out anything else on Schedule A if you check the box next to line 13. Compute the percentage of income that comes from public support in Section C. Check the box next to the line that describes the percentage of support you are getting. If you cannot check any of the boxes, check box 18 and change your reason for being a public charity to the one listed on line 9.

Provide information about your support schedule in Part III only if you checked the box next to line 9. Report information about any unusual grants or contributions that are not reported as revenue. Make a list for your records of all people who contributed money to your organization, but do not attach this list to the form.

Write any additional information about your organization or contributions to it in Part IV.

Attach Schedule A to your IRS Form 990 or 990-EZ.
