- 1). Print a copy of the Social Security Administration Application for a Social Security Card from SSA.gov on letter sized paper; legal-sized paper is not allowed.
- 2). Fill out the application completely, using black or blue ink only.
- 3). Gather your birth certificate and one form of government-issued photo identification, such as a driver's license, state-issued identification card or passport. If you do not have photo identification and you can't obtain it within 10 days, the Social Security office may accept a U.S. military ID card, an employee identification card, Certificate of Naturalization, certified copies of medical records, a Medicaid card, health insurance card or school ID card. Replacement cards for young children require the birth certificate and medical records from a doctor, hospital or clinic. If you aren't a citizen of the U.S., you must provide your U.S. immigration documents and foreign passport, with either a photo or biographical information.
- 4). Visit the Denver, Colorado Social Security office with your application and documents, or mail the application and documents to the main Denver office. The address is:

The documents that you submit to prove your identity must be either the original documents or copies that have been certified by the issuing agency--the SSA will not accept plain photocopies or even notarized copies. If you mail your application, your documents will be returned to you promptly.
