You must fill in your name, telephone number and email address, your electronic travel voucher number and the ticket number, booking reference number, flight number, travel dates and departure and destination locations of the flight ticket to which you want to apply the value of the voucher. Keep in mind that you can only apply the value of the voucher toward flight tickets for which travel has already been completed using this method.
