- 1). Obtain an application for a Social Security card (SS-5) online at the Social Security Administration's website or request an application through the mail by calling 800-772-1213.
- 2). Complete the application in blue or black ink. Provide all required personal information, including citizenship status, date and place of birth, phone number and mailing address. Sign and date the form.
- 3). Gather at least two original documents that prove your age, identity and citizenship. Acceptable supporting documents include a U.S. birth certificate or consular report of birth, current passport, a certificate of naturalization or citizenship and a driver's license or state ID. Immigrants must provide form I-551, I-94, I-766 or I-688B from the Department of Homeland Security and an unexpired passport.
- 4). Submit the application and supporting documentation in person at your local Social Security office if you are over 12 years old. Applications for children under the age of 12 may be submitted by mail. Original supporting documents sent with the application will be returned to you by mail.
- 5). Check your mail for your Social Security card. Cards are issued seven to 14 days after receipt of the application. Verify that your name is printed correctly and sign the card. Keep your Social Security card in a safe place to prevent fraudulent use of your Social Security number.
